Thrive18 was launched by the Northside's Project Destiny in 2018 to help residents overcome the barriers that prevent them from receiving healthcare and leading healthier lives. These social determinants of health barriers include food insecurity, lack of transportation, inadequate housing opportunities and, inability to access childcare and many more.
Transforming access, changing lives.
By using community health workers to identify and help develop solutions to overcome these barriers, as well as partnering with Highmark WholeCare, Allegheny General Hospital, Allegheny County's Department of Human Services and Health Department and the City of Pittsburgh, Thrive18 has created a sustainable pathway to better health outcomes for thousands of Northside residents - and is working tirelessly to help many more.

Utilizing a Community Health Worker (CHW) model, Thrive18 addresses social determinants of health—such as food insecurity, housing instability, education, transportation, and utility needs—by connecting residents with appropriate resources and services. The program aims to stabilize health by meeting basic needs, improve overall well-being, and support vulnerable populations through outreach efforts and a centralized wellness center.
Thrive18 serves residents of Pittsburgh’s Northside, specifically those living in ZIP codes 15212, 15214, and 15233. The program is free of charge and focuses on assisting individuals and families facing social barriers to health. Community Health Workers, often residents themselves, engage directly with the community to identify needs and facilitate access to necessary services.
Northside residents seeking assistance can contact Project Destiny at 412-231-1258, a line that operates 24/7 to address community needs. Community Health Workers are also active in the neighborhoods, conducting door-to-door outreach to connect with residents. Additionally, Project Destiny offers a Referral/Intake Form on their website for those who prefer to request services online.
Thrive18 helps Northside residents address a wide range of basic and social needs that impact health and well-being. These include food insecurity, housing instability, utility shutoffs, lack of access to healthcare, transportation challenges, mental health concerns, and support for youth and education. Community Health Workers work one-on-one with residents to connect them to the right resources and services at no cost.
